Statement of problem. This dissertation addresses the problem of need and use of mental health services for depression, alcohol dependence, social phobia, panic disorder, and agoraphobia, from five different perspectives. First, it will provide an estimate of the need for mental health services in the community, independent of diagnosis, and compare it to estimates based on prevalence data. Second, it will provide data demonstrating the contrast between those who use and those to do not use mental health services when they meet criteria for a specific disorder. Third, it will assess how insight affects the likelihood of using services for mental disorders. Fourth, it will measure the sensitivity and specificity of the Global Assessment of Function (GAF) in identifying need for services. Fifth, it will investigate two possible sources of bias in samples of individuals which originate in treatment facilities: the role of psychiatric comorbidity; and the role of family history of mental disorders in use of services for mental health.

Methods. Board certified psychiatrists interviewed a probability sample of the Baltimore Epidemiologic Catchment Area (ECA) population. The Schedule for Clinical Assessment in Neuropsychiatry (SCAN) was used. After the SCAN, the psychiatrist reviewed information available from the Diagnostic Interview Schedule (DIS), which had been administered in the household prior to the SCAN. At the conclusion of the examination, combining all information available, the psychiatrist ascertained diagnoses, judged the need for services for each broad category of mental disorder, rated the degree of insight about each mental disorder, and estimated a GAF score. Probability weights were used to calculate population estimates taking into consideration the sampling procedure. Demographic variables, measures of functional impairment, severity of illness, and degree of insight, were compared between those using and not using mental health services. The sensitivity and specificity of different threshold levels of the GAF were calculated, and summarized in Receiver Operating Characteristic (ROC) curves.

Results. The population prevalence of need for mental health services was 28.7% (S.E. 2.7). Among the disorders studied, alcohol dependence presented the highest amount on need, with a population prevalence of 13.6% (S.E. 1.9), followed closely by major depression, with 10.5% (S.E. 2.1). Population estimates of need for services for social phobia were 5.2% (S.E. 0.9), followed by 5.2% (S.E. 1.6) for panic disorder, and 3.2% (S.E. 0.9) for agoraphobia. (Abstract shortened by UMI.)
