This study investigates the potential change in the line of sight after laser in-situ keratomileusis (LASIK) by comparing treated eyes and untreated eyes. Corneal topography was retrospectively examined in 36 eyes of 22 patients measured before and after LASIK. The 36 eyes were divided into a high myopia group (n = 15) with correction more than 6 D and a low myopia group (n = 21) with correction less than 6 D. A control group comprised 18 untreated eyes of 9 patients with repeated topographic measurements to compare with preoperative to postoperative differences in the treated eyes.

It has been theoretically shown that the apparent pupil offset as measured on corneal topography is related to line of sight. The comparison between the treated and untreated eyes shows that LASIK changes the line of sight. A mathematical model was developed to simulate the anterior and posterior corneal surfaces. The distance between the two surfaces was adjusted to simulate the thickness of the cornea. The model uses translation and rotation of the cornea to simulate the effects of possible fixation change. Pachymetry maps show the thickness of the cornea. The difference map generated by subtracting the postoperative pachymetry map from the preoperative pachymetry map should highlight the ablation area. The model confirms that lower correction has more displacement on the pachymetry difference map. The position of the maximum difference point is related to the distance between the thinnest spot of the preoperative pachymetry map and the thinnest point of the postoperative pachymetry map.

The change of line of sight implies that refractive surgery still has room for improvement. Corneal topography is a tool to evaluate refractive surgery. If, in the future, corneal topography systems can take into account the change of line of sight after surgery, both the accuracy of measurement and the predictability of surgery can improve to benefit patients.
