Changes in mental health services have brought service delivery systems into conflict with many psychotherapists' methods of practice. Despite practitioners' key role in service provision, little is known about the relationships between service contexts, practice models, and therapist effectiveness. The aim of this pair of studies was to articulate mechanisms by which workplace features, in combination with therapists' models of treatment, produce outcomes relevant to service provision.

The aim of this pair of studies was to articulate mechanisms by which workplace features, in combination with therapists' models of treatment, produce outcomes relevant to service provision.

Participants included 8 interviewees and 453 questionnaire respondents who were APA or NASW members in clinical practice in Massachusetts. The Models of Treatment Questionnaire (MOT) was designed to assess approaches to therapy. Principal components analysis yielded 4 internally consistent subscales: teach/coach, fix/solve, and creative/philosophical treatment approaches, and negative emotional responses to therapy. The Workplace Features & Therapy Practices Questionnaire (WFTP) was developed to assess managed care workplace features. Hypotheses were examined regarding relationships between therapists' models of treatment, managed care workplace features, role conflict, and burnout. Results indicate that: (a) role conflict and negative emotion mediate the relationship between managed care and emotional exhaustion, (b) task orientation and negative emotion predict depersonalization, (c) negative emotion predicts low personal accomplishment, and (d) creativity orientation predicts high personal accomplishment. Work environment may be associated with therapist outcomes through negative affect. Results suggest interventions to ameliorate burnout should target environmental pressures and improve therapists' coping with negative emotional reactions to the therapy process.
