The diets of most children in the United States need improvement. Because children eat at least one meal per day at school, it is important to optimize the selection available to them at school meals and to promote the choice of healthier options. In the Shape Up Somerville - Audio Adventures study, we introduced two new low-fat, high fiber dried bean dishes into the school lunch menu. We then examined the effect of promotional audio messages broadcast over school public address systems on mediating variables and on choice of the new dishes.

Food advertising is effective in changing preferences and behavior of children. A content analysis of food advertising was undertaken to determine the techniques and persuasive strategies that are used. Data from the content analysis and from focus groups conducted with children in the target population were used to develop the audio messages.

To test the effectiveness of the school public address system as a communications channel to promote behavior change, six elementary schools were pair-matched and one school from each pair was randomly chosen to play the messages. Children's choice of the new dried bean dish was determined for each of the schools by direct observation. Using a questionnaire that was administered pre-and post-intervention, we also assessed the impact of the intervention on knowledge, attitudes, beliefs, social norms, preference, and intention to choose the new dishes. Data were analyzed using the generalized estimating equation (GEE) method for binary data with logit link function. The regression model accounted for the school as the unit of randomization. In addition to outcome data, extensive process evaluation data were collected to determine context, reach, dose delivered, dose received, and fidelity to the implementation plan.

For all schools combined, choice of the new dish was not significantly affected by the messages. However, process evaluation data indicated that the schools did not all deliver the dose of messages that was intended in the intervention plan. Compared to their matched control schools, choice of the bean dishes was significantly higher in the school that received the highest dose of the messages, and was significantly lower in the school that received the lowest dose. Choice was not changed in the school that received an intermediate dose. Although some psychosocial variables were significantly changed by the intervention, the results were variable and not always in the expected direction. These results suggest that school public address systems show promise as an effective and appropriate communications channel in schools that are able to deliver a high dose of messages.
