Among homeless women violence is a pervasive aspect of life. History of violence, mental illness, substance abuse, and lack of social support have been associated with a higher risk of suicide.

520 $a Objective. To describe a sample of homeless women, and to explore suicidal thoughts or attempts experienced by them. The relationship between history of harm and suicidality is described in addition to other correlates.
520 $a Design, setting, and participants. A cross-sectional survey of 105 homeless women randomly recruited from women residing overnight in the two largest adult-only, co-ed homeless shelters in San Francisco, California was conducted over a six-month period. Secondary analysis of this data forms the basis of this study.
520 $a Main outcome measure. Self-reported suicidal attempts and suicidal ideation, both 30-day and lifetime.
520 $a Results. The women in this sample were 41.3 years old on average (median 42) and had been homeless an average of 3.2 months. Fifty-eight percent had either thought seriously about suicide or made a suicide attempt within their lifetime. Twenty-three percent were restricted to suicidal ideation, and 35% had suicide attempts. Within the past 30 days, 18% had thought seriously about suicide and 1% had made an attempt. Of the women sampled, 44% reported a history of harm in childhood; 60% reported that they had been harmed by a stranger; and 64% reported that they had been harmed by a partner.
520 $a Those who reported a history of harm were over 4 times as likely to have had lifetime suicide thoughts or attempts, and this variable remained significant while controlling for other variables (AOR 4.4, 95% confidence interval 1.2--16.9, p = .030).
520 $a Perceived health status (p = .050) and the BDI score (p = .003) were associated with 30-day thoughts or attempts. History of harm showed similar trends as seen in the analysis of lifetime thoughts and attempts.
520 $a Conclusions. These findings underscore the need to not only to actively screen homeless women for suicidal ideation, but also to identify mediating factors associated with suicidal thoughts and attempts.
