Academic Medical Centers (AMCs) are an integral part of the health system in the US In recent years AMCs have experienced significant financial and operational problems. Capable leadership is necessary for organizations to function effectively. This is a phenomenologic study exploring facets of leadership at AMCs. Chairs of departments of Internal Medicine were the focus of this study and ten Midwestern chairs were the participating subjects. Data were collected between February, 2004 and June, 2004. Three structured, open-ended interviews were conducted and two instruments assessing self-efficacy and job content were administered. Curriculum vitas were also analyzed. Qualitative software was utilized to analyze the emergent themes. Relevant findings were around nine main themes: (1) Successful chairs have had a significant amount of administrative experience; (2) leadership skills are essential and Emotional Intelligence Competencies (EIC) were the most important skills identified; (3) the job of chair and the application of leadership skills is complex and difficult; (4) the selection process for selecting chairs is fatally flawed; (5) mentoring is the most prevalent method of developing leadership skills; (6) organizational conditions, especially the relationship with the CEO of the hospital, are important factors for effective leadership; (7) educational involvement provided a great deal of satisfaction; (8) work-life balance is of great concern to chairs; and (9) the lack of congruency in expectation and leadership skills are the most important reasons chairs fail. Recommendations include: (1) redesigning the selection process; (2) improving methods of mentoring and providing leadership development through learning collaboratives; (3) embracing the field of organization development as a partner for solving organizational issues; (4) establishing an institute for Academic Leadership to address these problems; and (5) developing a focused research agenda which explores methods of improving leadership at AMCs.
