This study investigated barriers to mental health care utilization among 69 low-income, English and Spanish-speaking pregnant and postpartum women with depression who were referred for treatment. Results showed 77% (n = 53) women did not attend an initial mental health appointment, and pregnant women were significantly less likely to attend treatment than postpartum women. Among those who did not attend treatment, environmental/logistical and attitudinal barriers were the most frequently cited barriers to treatment; increased age, lack of insurance, and a prior diagnostic history were predictive of environmental/logistical barriers; earning a high-school diploma and not having engaged in treatment in the past were predictive of attitudinal barriers; and postpartum status and not having been given a previous diagnosis were predictive of encountering agency barriers. While sharing some similarities with the literature regarding treatment engagement, these findings also offer new insights into the factors related to treatment underutilization among pregnant and postpartum women.
