School-based health centers (SBHCs) have become an important source of health care for millions of school-aged children and adolescents. These centers were first established in the late 1960s and early 1970s to provide and ensure access to quality health care for all young people in school, regardless of socio-economic status. These centers bring a range of needed health care services, including services around mental health care, to children and youth in a familiar and convenient environment. During the 2001--2002 academic year, there were 60 centers across Maryland serving children and teens in urban, suburban and rural areas; 46 of these centers included a mental health care program providing services to disadvantaged children and youth to help address the mental health problems that are too often detrimental to their overall health and well-being.

Using data from the 2001--2002 Maryland School-Based Health Center Survey, this study examined the association between demographic, staffing, operational, utilization, and policy factors and the availability of 14 mental health services on-site in Maryland school-based health centers. The on-site presence of a licensed social worker or counselor significantly increased the odds of offering mental health diagnosis and conflict resolution/mediation services on-site. The likelihood of providing medication management and administration services on-site was found to be associated with the on-site presence of a licensed social worker or counselor, the center being used as a primary care home, and the center allowing students to access mental health services without parental permission. Offering the ability to access mental health services without parental permission also increased a center's likelihood of providing medication follow-up services on-site. The availability of substance use counseling on-site was found to be associated with access to mental health services without parental permission and having a mental health program at the center, while the provision of tobacco counseling services was associated with school level, being able to access mental health services without parental permission, and having a mental health program at the center.

These results indicate that certain staffing and policy-related factors---most notably the accessibility of mental health services without parental permission and the on-site availability of a licensed social worker or counselor---play an important role in the on-site provision of several mental health services in Maryland school based health centers. This study adds to the existing literature on the provision of school-based mental health services, particularly from an organizational perspective.
