Quality of life [QOL] has become an essential outcome measure for chronic mentally ill patients since deinstitionalization in 1984 in Taiwan. The overall objective of the study was to compare QOL in Taiwanese schizophrenic outpatients from day care settings with comparable schizophrenic outpatients from home care setting. This study was a cross-sectional comparison design. A purposive sampling of 70 subjects was drawn from the day care settings of one medical center (n = 25) and one psychiatric hospital (n = 20), and the home care setting of one psychiatric hospital (n = 25). QLSP, PANSS, and demographic profiles were obtained from in depth-interviews, self-reports and patients' medical records.Subjects--Topical Terms:

Different levels of psychopathological statuses were identified in the three settings. Schizophrenic patients in day care setting had significantly higher level of satisfaction in daily life than those in home care setting, especially in the role function, family life, and social & cultural life domains. Schizophrenic patients in day care setting had a better living environment in objective QOL [OQOL] than those in home care setting. Psychopathological statuses were as complex relationships with subjective QOL [SQOL], and were inversely correlated with OQOL in schizophrenic patients. Females tended to have a higher level of satisfaction with their living environment, role function, and family life; and to have a better performance in family life. The single persons were more dissatisfied with their physical health and had a worse performance in their role function. Schizophrenic patients who were employed tended to have a better performance in their role function.

The type of care setting was predictor, accounting for 8.9% of variance in SQOL. Psychopathological statuses were predictors, accounting for 9.2% of variance in OQOL of schizophrenic patients.

The study concluded that both day care and home care services are benefits for overall QOL of schizophrenic patients, and policy makers and health providers should acknowledge the essence of day care programs for the chronic mentally ill. In working with schizophrenic patients we should aim at relieving psychopathological symptoms as well as provide rehabilitation programs that offer substantial social interaction, occupational training and employment opportunities.
