The Arab region has entered an era of dramatic demographic shifts. Declining fertility is one of the most significant of these changes. This transition has been largely the outcome of changes in contraceptive use. As contraceptive prevalence continues to rise, it is important to gain a deeper understanding of contraceptive use in the region. This dissertation takes the form of five distinct, though logically-interrelated, papers. I am particularly interested in the following issues: (i) the role of contraception as a key determinant of fertility change; (ii) inequality and changes in the patterns and causal factors affecting contraceptive use and method choice; (iii) levels of mistimed and unwanted pregnancies, the extent of their repeatability and contributing factors; (iv) strength of fertility motivations and their impact on unmet need and contraceptive choice; and (v) measurement and assessment of quality of care in family planning services. Specifically, I will focus on the experiences of Egypt, Jordan and Morocco.

Based on secondary analyses of data from Egypt and Jordan, Chapter II shows that contraception is one of the key determinants of fertility change in these two countries. Even when contraceptive use increased remarkably and some persistent contraceptive gaps narrowed over time, findings from Chapter III show that Egypt, Jordan and Morocco face various obstacles to reducing unmet need and averting unintended childbearing, including limited contraceptive choice and persisting differentials in contraceptive use and choice by geographic region. Notably, unintended pregnancies are not rare events: More than two in ten pregnancies in Egypt, and three in ten pregnancies in Jordan and Morocco, were classified as unintended. The importance of addressing the strength of women's fertility motivations is examined in Chapter V. At least a quarter of Egyptian women and one in three women in Jordan and Morocco appear to be ambivalent about future childbearing. These women are significantly less likely to use contraception or to rely on inefficient methods. The last chapter demonstrates the applicability of the Egypt Service Provision Assessment instrument as a method for assessing quality of care. There are a number of areas where preparedness and quality of care should be improved, including the number of methods discussed with clients and the provision of information on potential side effects and their management.
