The purpose of this study was to investigate intrapsychic and psychosocial aspects of HIV-seropositive substance abusers that facilitated or hampered their disclosure to health care providers on issues relating to personal problems and feelings, health care and lifestyle. It was hypothesized that difficulty of self-disclosure was a function of the patients' perceived empathy of their health care provider, their degree of alienation and their health locus of control. A secondary objective of this study was to examine the connection between reported difficulty of self-disclosure and compliance, in the expectation that patients who had less difficulty communicating with their health care providers would tend to show up more regularly for scheduled appointments.

Data was collected from 122 HIV-seropositive male patients registered at a veterans administration hospital, with a documented history of substance abuse. Subjects completed a demographic questionnaire plus a series of instruments designed to measure difficulty of patient self-disclosure as the dependent variable, perceived empathy of the health-care provider, alienation and health locus of control as the independent variables.

Results of hierarchical multiple regressions showed perceived clinician empathy and internal health locus of control to be significant predictors of patient self-disclosure; alienation emerged as a significant predictor in only one of the self-disclosure subscales, and two subscales of health locus of control produced no significant results. Two demographic variables, race and subjective health status, emerged as significant correlates of patient self-disclosure and were entered into the regression analyses to control for overlapping variance with key predictor variables in the hypotheses. Finally, a discriminant analysis showed that reported difficulty of patient self-disclosure successfully predicted differences in group means between compliant and noncompliant patients.
