There is a growing awareness that the parturient subjected to the minimum medical intervention and afforded the maximum autonomy and self-control evidences the most positive (lowest complications, highest satisfaction) childbirth. Newer, more natural birth systems (ABC)'s which reflect the above philosophy have been instituted and this has prompted preliminary studies into the efficacy of the ABC birth when compared with the more traditional Lamaze-prepared Labor and Delivery Suite (L&D) birth.Subjects--Topical Terms:

In the present study assessment of the ABC versus L&D birth and psychological and psychosocial variables as they relate to the physiological and emotional well-being of mother and infant generated three multivariate regression/correlational analyses design. Subjects were 54 predominantly white, middle-income, Lamaze-prepared primaparae, 44 of whom were low risk, 10 of whom were high risk and assessed separately. Subjects provided measures of anxiety, health locus of control orientation and choice of ABC or L&D birth setting during the third trimester of pregnancy testing. Postpartum, subjects provided measures of (1) stress experienced during pregnancy; (2) complications, degree of satisfaction, and perceived control experienced during childbirth.

The primary findings are that those subjects afforded the most natural of births with the greatest reported staff supportiveness evidence less complications and more satisfaction than their L&D counterparts. Additionally, Internal locus of control orientation is positively linked to degree of satisfaction. Further, when stress-related anxiety is held constant, an inverse relationship is found between internalized, trait anxiety and complications. The speculation is the more denied the anxiety, the higher the complications.

Although the above variables accounted for 43% of the variance in childbirth complications, there is still much left to know about different sources of pregnancy and childbirth anxiety. Also there is much to know about the possible role of the obstetric staff's support in facilitating the parturient's ability to deal with anxiety, and about the role of expectations to expressed satisfaction or dissatisfaction with the process of childbirth. Continued investigation in this area is warranted to further elucidate variables such as the above which have the potential of helping or hindering the process of birth.
