Health care costs have escalated tremendously in recent years fueled in part by mistakes made within the health care system, expensive new technologies for diagnosis and treatment, and added costs due to governmental regulations and litigation. As such, audits, controls, compliance, and administration are paramount to alleviate the problem. Patient safety and proper management of patient information requires innovative identity management systems. In health care, authentication and authorization are a priority before a given task or procedure is undertaken. This research incorporates a new identity management approach for use in health care systems as a partial solution to these issues.

Validation of credentials for many procedures in health care have a very low threshold for mistakes or imprecision, but there are also many procedures that are non-critical and can be more flexible. As part of this research, an initial object-oriented, role-based identity management system for health care procedures was first created. This simple identity management approach was then expanded to include fuzzy reasoning as a method of providing flexible authentication and authorization for non-critical procedures. This research also indicates how RFID and biometrics can be used as additional enhancements to the identity management mechanism.

Finally, a prototype next-generation health information system, known as the NHIS system, was developed and tested to incorporate the new identity management approaches developed in this dissertation. This prototype utilized an extensive 75,000 patient dataset from the Kentucky Hospital Association (KHA). Testing of the NHIS prototype using the KHA database gave promising results for this enhanced identity management approach for health care applications.
