This study compares the effects of doula support at birth and Lamaze birth preparation in a group of first-time pregnant mothers. Through the use of an interview (George & Sotomon, 1994; Orenstein, 1995) the mothers' working models of caregiving were assessed during pregnancy and again post partum at four months. In addition, measurements of mood state and self-esteem were given at these times. Finally, a post hoc analysis was done on the mothers' birth experiences, the quality of their post partum support, and their evaluations of their infants' temperaments. It was hypothesized that the doula group, more than the Lamaze group, would demonstrate a positive change on these measures. It was also hypothesized that measures of working models of caregiving would be postively associated with measures of emotional state and self-esteem.

The results partially confirm these hypotheses. The pre- to post-birth rating analysis of the interviews revealed the doula group to be significantly less rejecting and helpless in their working models of caregiving. They also showed an increase in security which approached significance, but no change in pre- to post-birth uncertainty. In addition, they were less emotionally distressed and had higher self-esteem than the Lamaze group which showed a trend in the opposite direction, although these findings did not reach significance.

As predicted, security of caregiving was positively associated with less mood disturbance and higher self-esteem. Pre-birth secure mothers from both groups measured significantly less tense and less angry than insecures on the Profiles of Mood State (POMS). Post-birth secures were significantly less disturbed in mood than insecures on all scales of the POMS. Similarly, post-birth secures had significantly higher self-esteem than post-birth insecures as measured by the Rosenberg Self-Esteem Inventory.

A process model of attachment formation is used to argue that the doula's support of the mother's competence during birth provides her with an experience of confidence in her caregiving capability that has a positive influence on her post partum views about caring for her child, her emotional state and her self-esteem.
