This focused ethnographic study explored the nature of trust between nurses and patients in a psychiatric inpatient unit. The sample included 15 registered nurses and 22 adult patients. Trust between nurses and patients was examined from the perspective of the patient's trust of the nurse and the nurse's trust of the patient. The findings delineated characteristics of trustworthiness and revealed that trust is context-dependent. Patients viewed the maintenance of confidentiality, except in situations of potential harm to self or others, as essential for their trust in the nurse. Nurses emphasized patient compliance with treatment expectations as the primary indicator of trust.

The nurse's trust of the patient was found to be less important to the therapeutic relationship. Trusting patients was often perceived to be inconsistent with safe nursing practice. Thus, the suggestion that reciprocal trust is important to the nurse-patient relationship (Thorne & Robinson, 1988) was not supported by this study.

Facilitators of trust were delineated as "connecting," "comforting," and "coaching" and encompassed nurses' actions and attitudes that promoted a trusting relationship. The study also explored factors that inhibited the formation of trust and found no congruence between nurse and patient perceptions. Although the study addressed nurse-patient trust, trust among staff emerged as an important aspect of working with psychiatric patients.
