A double-masked, controlled, randomized, clinical trial was performed to determine if a diet formulated specifically for cats with CKD is superior to an adult maintenance diet in minimizing uremic episodes and mortality in cats with spontaneous stage 2 or 3 CKD. Forty-five client-owned cats were randomly assigned to an adult maintenance diet (23 cats) or a renal diet (22 cats) and evaluated tri-monthly for up to 24 months. The renal diet evaluated in this study was superior to an adult maintenance diet in minimizing uremic episodes and renal-related mortality in cats with spontaneous stage 2 or 3 CKD.

A case control study was performed to retrospectively determine if the presence of nephroliths was associated with increased mortality or progression of disease in cats with stages 2 and 3 naturally occurring CKD. The clinical course of nephroliths in seven cats with CKD was compared to an age and diet matched group of CKD cats without nephroliths. Complete evaluations were performed tri-monthly for 24 months. Cats were enrolled in the study between April of 2000 and March of 2003, and survival data was collected until February 16, 2006. Differences in clinical or laboratory parameters, or in the incidence of uremic crises, renal-related mortality, or all causes of mortality were not observed between the two groups. These findings corroborate the current consensus of opinion that favors conservative, non-surgical management of non-infected, non-obstructive nephroliths.

In summary, our results indicate that a diet modified for cats with kidney disease reduces the occurrence of uremic crises and renal-related death in cats with stage 2 or 3 CKD. Our results also support conservative management of nephroliths in cats with stage 2 or 3 CKD.
