The purpose of this study was to evaluate whether an occupational therapy intervention improved an academic outcome (D'Nealian printing) in a school setting. The study specifically examined improvement in printing skills in academically at-risk first graders. The intervention was based on a developmental model supplemented by biomechanical, acquisitional, and sensory-motor theories. Fifty-nine first grade children from a low socioeconomic urban elementary school-based health center were randomly assigned to an experimental (occupational therapy intervention) or control (no intervention) condition. Following confirmation of test-retest and interrater reliability of the Minnesota Handwriting Test (MHT), all children were tested on the MHT before and after the intervention phase of the study. The MHT measures reproduction skills necessary for accurate printing (legibility, space, line, size, and form), which were the dependent variables of this study. The MHT also measures speed, but this variable is not a consistent measure of quality. Raters were not aware of group placement.

In addition to regular academic instruction the experimental group received 10 weeks of handwriting intervention twice a week for 30 minutes. Fourteen occupational therapy students trained and supervised by the principal investigator conducted the intervention. The control group received only regular academic instruction.

Multivariate analysis of variance (MANOVA) confirmed that the intervention group improved on the main five variables under study when compared to the control group condition (<italic>p</italic> < .0001). Secondary univariate analyses of variance demonstrated intervention effects for space, line, and size at the <italic>p</italic> < .01 alpha level. Therefore, the occupational therapy intervention has been demonstrated to be effective in improving printing skills in at-risk first grade students.
