For Part One of the study, 189 hospital patient charts having specialized x-ray studies or surgery were reviewed for Bankart and Hill-Sachs lesions and for history and examination findings associated with instability characteristics. For Part Two, baseball, softball and cross-country athletes completed a shoulder injury questionnaire (SIQ), shoulder rotation motion measures, and isometric rotation peak torque measures. Isometric peak torque was measured at 30 degrees of internal and of external rotation by use of a CYBEX 6000. All measurements of rotation were made with the subject supine, the shoulder abducted to 90 degrees, and the arm brought into the plane of the scapula.

Bankart-type or Hill-Sachs lesions were present in 29 (74%) of the 39 atraumatic shoulder instability patients. There were no significant differences in either lesion occurrence between males and females. Lesions in both genders were not associated with any specific instability characteristic. Twenty-six Runners (6 of 16) and Throwers (20 of 37) showed positive instability characteristics, but there was no significant differences in the expected frequency between those with and those without such characteristics. Instability characteristics in Throwers were positively correlated (r = 0.40) with the number of seasons of play. There were no detectable shoulder range of motion differences nor isometric peak torque differences between Runners and Throwers.

Bankart or Hill-Sachs lesions were present in the majority of 18-28 year old, atraumatic, subluxation patients who sought medical advice. However, the presence of either shoulder lesion was not consistently associated with instability complaints. Athletes who are Runners have similar frequencies of instability complaints as those athletes who are Throwers. These complaints are similar to young instability patients. No difference exists between Runners and Throwers in their shoulder rotation motion and isometric strength that would help to identify those with or without instability characteristics.
