A population-based case-control study of 486 male and female leukemia residents of urban Shanghai and 502 healthy controls was conducted during 1987 to 1989 to evaluate the association of lifestyle risk factors and occupational exposures with the risk of leukemia. Cases were identified from the Shanghai Cancer Registry. Controls were randomly selected from the general population of the city using the comprehensive, population-based Resident Registry in a two-step selection process and frequency matched to cases by age and sex strata.

Information was obtained from 486 (91.4%) of the 532 eligible cases and directly ascertained from 254 living cases (52%) and 232 (48%) next-of-kin cases (194 deceased, and 38 ill). Interview information was obtained directly from 492 of the 502 controls (98%) and from next-of-kin for 10 controls (2%). Cigarette smoking was not found to be associated with the risk of leukemia (OR = 0.90, 95% CI = 0.65–1.23). The risk associated with smoking was not elevated for all four major cell types of leukemia with ORs ranging from 0.57 to 0.98. No dose-response relation was seen between risk of leukemia and the duration of smoking, number of cigarettes smoked per day and pack-years of smoking. Alcohol was not found to be associated with the risk of leukemia (OR = 0.88, 95% CI = 0.61–1.25). Ever use of hair dye was associated with significantly increased risk for CIVIL in all subjects and all leukemias among males. The results associating the use of hair dye to leukemia lends support to the results of some other studies that have suggested hair dye use as a risk factor for leukemia. A job-exposure matrix (JEM) based on the industrial and occupational headings defined for the 1982 Chinese population census was used in addition to self-report to assess occupational exposure. JEM exposure probabilities were none, low, medium, or high. Agreement between self-reported exposure and JEM-assessed exposure was evaluated using kappa coefficients in all participants, direct-interviewed subjects, cases, and controls subgroups separately. The kappa coefficient among the subgroups ranged from 0.58–0.67 for benzene, from 0.75–0.79 for ‘other organic solvents’, from 0.70–0.77 for EMF, and was 0.83 for pesticide. This suggests that the self-report exposure agreement with the JEM was good and that the conclusions from the results would be the same regardless of the subgroup that was studied.

The risk for leukemia was increased significantly among chemical manufacturing workers (OR = 3.13, 95% CI = 1.00–9.78). Risks for leukemia were also increased significantly among subjects ever exposed to benzene (OR = 1.73, 95% CI = 1.13–2.64), radioactive materials (OR = 3.65, 95% CI = 1.30–10.22), synthetic fiber dust (OR = 2.04, 95% CI = 1.19–3.49), and toluene. Elevated risk of leukemia for all major leukemia types were found among those who were exposed to benzene and radioactive materials, although the strength of the association was strongest for CML.
