Different aspects of medical resource utilization are found to be influenced by different factors. By adding provider variables the forecast performance of the predictive model is also improved.

In contrast to previously developed predictive models, this study uses both variables measuring consumer characteristics and variables measuring provider characteristics simultaneously as predictors.

The model is estimated from the data of a large HMO in the State of Minnesota with a total of 61772 individuals who were continuously enrolled for the period of 1991-1992.

Three behavioral models of consumers and providers are constructed using the theoretical framework of utility-maximization. Based on the behavioral models a Four-Part predictive model of medical resource utilization is developed.

The process of medical resource utilization is viewed as consisting of three distinct components involving different decision makers: the consumer's decision to seek medical care, the physician's decision to hospitalize a patient, and the physician's decision on the treatment intensity.

A certain portion of the previously unexplained inter-patient variance in medical resource use, which would be otherwise attributed as purely "random" by models with only consumer variables, is found to be associated with different characteristics of the patient's providers.

The HMO member's age, gender, prior utilization, number of prior diagnoses, and personal characteristics of the member's principal physician, the number of technologies at the provider's disposal, and the use of practice guidelines are identified as strong predictors of medical resource utilization. Policy implications and implications for future research are discussed.
