<italic>Background</italic>. The relationship between bereavement and psychopathology has not received as much attention in the epidemiologic literature as the physical health risks associated with the death of a loved one. The most studied diagnostic entity in this relationship is depression and recent epidemiologic work on this topic has directed attention to its public health significance. Yet, the generalizability of study results is often limited and unstandardized measures are frequently used to define outcomes. Potential moderators of this relationship are also understudied. The plausible role of individual religious involvement as a moderator of bereavement-related depression has been overlooked in spite of the fact that a majority of the population draw upon religious and spiritual resources for self-care and support during times of significant loss or crisis. Therefore, the objective of this study was to investigate the association between bereavement and depressive syndrome and to assess the influence of religious involvement on this relationship in a community sample of adults bereaved in the year prior to interview.

<italic>Methods</italic>. The Diagnostic Interview Schedule (DIS) was used to detect depressive symptomatology in 1,730 of the 1,920 persons classified by bereavement status in the 1993–96 Baltimore Followup of the NIMH-sponsored Epidemiologic Catchment Area (ECA) Survey. Depressive syndrome was the outcome of interest and was defined as the co-occurrence of an episode of sadness or loss of interest nearly every day, and the presence of one or more other DSM-III-R symptom groups, for a period at least two weeks in the year prior to interview. Data on religious beliefs and practices were obtained on respondents for whom there were complete bereavement data. The underlying structure of the religion variables was assessed using exploratory factor analysis. The solution was used to generate indicators of religious involvement that referred to frequency of religious attendance, level of spiritual support, and evangelical religious orientation. Individual respondents were assigned a bereavement risk score that was constructed from available information on three key aspects of each reported death. Logistic regression analyses modeled hypothesized associations between depressive syndrome and bereavement risk status, selected sociodemographic characteristics, the availability of a confidant, and the religion factors in these cross-sectional data.
