Since 1995, the Advisory Committee on Immunization Practices (ACID) has recommended that the men who have sex with men (MSM) community receive the hepatitis A vaccine in order to prevent further hepatitis A outbreaks. Unfortunately, these recommendations have not been widely implemented and outbreaks of hepatitis A virus (HAV) have continued in this community. According to recent surveillance reports from the Centers for Disease Control and Prevention (CDC), there has been an increase in the number of reported hepatitis A cases in the MSM community. During 1997, 914 cases of hepatitis A were reported in New York City, a 32 percent increase in cases from 1996. More than 50 percent of these cases occurred in Manhattan. Many of these cases may be linked to the increased number of sexual contacts and the “safer” sex practices adopted in the MSM community.

The purpose of this study was to ascertain the knowledge, beliefs and attitudes among the MSM community in New York City regarding hepatitis A infection and vaccination, and to discern which media sources are used in the community, using a convenience sample of 468 subjects recruited from neighborhoods where the 1997–1998 New York City hepatitis A outbreak occurred.

Based on the Logistic Regression, given an array of predictors, vaccination status was predicted very strongly with a 16 times more likelihood based on a prior history of vaccination for hepatitis B. Whether or not a person was infected or not infected with hepatitis A was predicted significantly by knowledge, having insurance, and age. People who were infected with hepatitis A were slightly older and less likely to have insurance. A low perception of risk was the primary barrier preventing participants from receiving the hepatitis A vaccination. Additionally, a lack of physician recommendations to receive the vaccine was another cause lowering the vaccination rate in the MSM community.

Based upon the results of this study, health educators need to focus on improving the accuracy of hepatitis A risk infection assessments in the gay community. In addition, an effective vaccination campaign should train health care providers and physicians to help people make those accurate risk assessments for themselves and get the hepatitis A vaccination.
