The study of health care provider fatigue is of crucial importance to the promotion of safe patient care environments. The daily demands of critical care nursing, including psychosocial stressors and heavy patient workloads in the ICU, make the nurse particularly vulnerable to fatigue, and subsequently accidents and errors. The purpose of this study was to explore shift-related differences in fatigue and the correlates of fatigue in a representative sample of critical care nurses.

The purpose of this study was to explore shift-related differences in fatigue and the correlates of fatigue in a representative sample of critical care nurses.

Using a correlational, comparative survey design, this study examined shift-related differences in fatigue and its correlates, including age, work shifts, sleep quality, anxiety, and depression among a nationwide, randomly selected sample of 142 female full-time bedside critical care nurses. The Standard Shiftwork Index, Pittsburgh Sleep Quality Index, Beck Anxiety Inventory, and Beck Depression Inventory-II were used to measure these variables. Questionnaires were mailed to 500 randomly selected members of the American Association of Critical Care Nurses and 247 nurses returned completed surveys. Chronic fatigue was negatively related to age (<italic>r</italic> = −.21; <italic>p </italic> < .01), and positively related to global sleep quality (<italic> r</italic> = .49; <italic>p</italic> < .01), sleep efficiency (<italic> r</italic> = .20; <italic>p</italic> < .01), sleep disturbances (<italic> r</italic> = .47; <italic>p</italic> < .01), anxiety (<italic>r</italic> = .46; <italic>p</italic> < .01), and depression (<italic>r</italic> = .63; <italic>p</italic> < .01). Independent t-tests revealed that night nurses experienced significantly more depression (<italic>t</italic> = −2.60; <italic> df</italic> = 140; <italic>p</italic> < .01), lower sleep efficiency (<italic> t</italic> = −2.24; <italic>df</italic> = 123; <italic>p</italic> < .01), shorter sleep duration (<italic>t</italic> = −1.94; <italic>df </italic> = 140; <italic>p</italic> < .05), and lower sleep quality (<italic> t</italic> = −2.94; <italic>df</italic> = 140; <italic>p</italic> < .01) than day nurses. Hierarchical multiple regression analysis revealed that among the variables of age, day or night shift, habitual sleep quality, depression, and anxiety, depression was the most important variable in the explanation of chronic fatigue (β = .45; <italic>t</italic> = 6.0; <italic>p</italic> < .001).

These findings suggest that depression and poor sleep quality are more prevalent in night shift nurses than day shift nurses. Depression and sleep quality are significant correlates of chronic fatigue. Educational and counseling programs to improve sleep and treat depression may be helpful in the reduction of chronic fatigue in critical care nurses. This is likely to result in improved health and safety for critical care nurses and improved patient care in acute care settings.
