A core feature of schizophrenia is the inability to appropriately regulate sensory information. A neural index of this is seen in abnormalities in the suppression of the P50 component of the auditory evoked potential. Normally, P50 is reduced in response to identical stimuli occurring rapidly in time (less than 1 second apart). In schizophrenia, P50 does not reduce as greatly to the successive stimuli. Across studies aimed at localizing P50 and its suppression mechanisms, disparate neural systems have been identified. However, recent data suggests that P50 may reflect the activity of multiple neural systems, thus bridging the disparities in the literature. This more complete understanding of the neural circuitry underlying P50 is critical for elucidating the nature of P50 abnormalities in schizophrenia. In this study, it was hypothesized that subcomponents of P50 could be identified that would be consistent with different neural generator systems reported in the literature: a midline subcomponent and frontal bilateral subcomponents.

The different systems implicated in P50 are also thought to be important for different types of attention. Attention is deficient in schizophrenia, and, as with P50, this is conceptually consistent with poor sensory filtering. Therefore, it was further hypothesized that abnormalities in the midline P50 subcomponent would predict poor performance on tests of sustained attention, considered dependent upon midline brain structures. It was also hypothesized that abnormalities in the bilateral subcomponents would predict poor performance on attention tests thought to be dependent on the frontal regions that appear related to bilateral P50 subcomponents. Analyses confirmed an earlier midline subcomponent and a later bilateral set of subcomponents over frontal regions. However, the midline subcomponent did not predict sustained attention measures. The left frontal subcomponent was more strongly related to the hypothesized attention measures compared with the right. The results of this study illustrate the complex nature of the P50 component, and demonstrate that an appreciation for this complexity can provide more thorough understanding of the relationship between P50 and other related phenomena observed in schizophrenia, thus unifying neural and behavioral observations into a more integrated picture of the illness.
