The Patient Video Interview (PVI) is an instrument designed to assess communication skills in medical education that overcomes some of the drawbacks of other assessment methods. Three studies were conducted to: (a) determine a measurement system for the PVI, (b) explore the overlaps between the PVI and other measures of communication skills in current use in medical schools, and (c) examine the relationship between a self-report measure of attitudes, values, and beliefs, and student performance on the PVI, as a means of exploring concurrent and predictive validity.

In Study 1, a number of issues were considered that are related to the measurement of medical communication in general and to the PVI specifically. These included issues of the relationship between categorization as a measurement strategy and mutual exclusivity. Both these topics were linked to that of inter-rater agreement, and ultimately resulted in the decision to use a system based on ratings rather than on coding categories.

Study 2 explored the relationships between the PVI, a videotaped standardized patient encounter (OSCE), and a preceptor evaluation of 33 students at one medical school. Modest overlap existed between the measures, demonstrating convergent and divergent validity of the PVI. Holding constant the measurement system, and thereby controlling for a significant source of variability, did not change the degree of overlap, but allowed for the emergence of significant correlations between the PVI and the OSCE.

Study 3 utilized 85 participants from a pool of 697 students at eight U.S. medical schools who had participated in both the PVI and a Core Survey designed to assess attitudes, values, and beliefs. This study examined concurrent validity between performance on the PVI and a set of medical student attributes and characteristics of current interest in medical education. These included associations with specialty choice, dealing with dying patients, the expression of empathy, the use of understandable language, and participation in the collaborative nature of medicine via referral to the interdisciplinary health care team. Significant relationships were found between the PVI and associations with specialty choice and dealing with dying patients.
