This dissertation evaluated whether dietary increases in cruciferous vegetable intake can reduce the onset of breast cancer as assessed by biological predictors. It was conducted in South Carolina using a randomized controlled study design in free-living individuals. The biological predictors were chosen for their prognostic significance: one being an indicator of circulating endogenous estrogen and the other a measure of oxidative stress. The purpose was to determine: (1) if women in the dietary intervention arm experience a mean group increase in the ratio of 2-hydroxyestrone to 16-hydroxyestrone; (2) if women in the dietary intervention arm experience a mean group increase in the oxidative stress biomarkers, 8-hydroxy-2'-deoxyguanosine and 8-iso-prostaglandin F2alpha; (3) whether motivation, nutritional knowledge or having a lower dietary fat intake is a stronger predictor success in a rapid dietary intervention.

In the first investigation, we found differences in individual estrogen metabolites (but not in the ratio) were affected by body mass index and that differences in the ratio were affected by physical activity but not by dietary intake. The effect of increasing cruciferous vegetable intake was not enough to show a difference in the levels of the individual estrogens after accounting for differences in body mass index.

In the last investigation we found that psychosocial predictors of attitude and behavior, dietary knowledge, and baseline dietary fat intake were not significant predictors of adherence or maintenance in this dietary intervention. This study showed that a rapid intervention can still be successful even if women are not highly motivated to change behavior.

There may be several confounders that need to be examined before determining if increasing cruciferous vegetable intake will result in concomitant changes in hormonal outcomes and oxidative stress outcomes. Future investigations should examine comorbid medication use, quality control in laboratory analyses, and biological levels of isothiocyanates before determining whether cruciferous intake can impact biological outcomes predictive of breast cancer.
