Over the past 15 years, a new paradigm for health promotion has evolved, focusing on the importance of community empowerment. Despite increasingly widespread acceptance of empowerment as an important strategy for, and an important outcome of, health promotion programs, the concept has yet to be clearly situated theoretically in relation to other outcomes of interest to health promotion, such as health-related behavior.

The current research elaborates a conceptual framework linking sociodemographic variables, empowerment, health behavior, and health. Using cross-sectional data from the evaluation of a community health worker (CHW) hypertension program, this research examined the relationship of individual empowerment to behavior and perceived health among African Americans in a low-income urban area. The impact of a community health worker program on empowerment was also analyzed.

A sense of coherence scale (Antonovsky 1993, 1987, 1978; Nyamathi, 199 1) was modified for use with this population as a measure of empowerment. Cognitive pretesting techniques and input from the CHWs were used to make changes in the response format and wording of questions.

Data used for this research were collected from 463 people, who were among nearly 800 people who had been randomized to either receive CHW visits (the intensive intervention group) or to receive only referrals and information (the minimal intervention group). In addition to empowerment and sociodemographic variables, the evaluation survey contained information on social support for hypertension control, worry about medication, hypertension compliance, body mass index (BMI), alcohol use, smoking, and perceived health.

Empowerment was negatively associated with smoking, smoking intensity, alcohol intake, and problem drinking. A positive association was found between empowerment and BMI. Empowerment was also positively associated with perceived health. No difference in empowerment was found between the intensive and minimal intervention groups.

Path analyses were conducted to investigate the possibility that empowerment acts as a mediator between sociodemographic variables and behavior, and between sociodemographic variables and perceived health. The path analyses for BMI and perceived health provided evidence for the possibility of this mediating relationship; in both models, age, having a disability, and unemployment had significant indirect effects on the outcome variables mediated by empowerment.
