The sample for this study comprised of 1,233 children (n=801 CPC; n=432 control) who had scores for one or more health outcomes by age-24 years.

This study explored the association between a large-scale public preschool intervention and the long-term health of low-income minority youth at age-24 years. The program's main effects were examined on respondents' daily smoking, frequent smoking, number of days per week youth had smoked a cigarette; substance misuse since age-16 years, and substance misuse with a conviction for drug possession; access to a doctor; and good health and poor health status. All outcomes were explored with one year and two years of preschool participation. Sub-group effects were investigated using child, family, and program moderators. The study also examined whether high school completion mediated the effects of preschool participation on health into adulthood.

Data were drawn from the Chicago Longitudinal Study (CLS), a quasi-experimental prospective evaluation. The original CLS sample of 1,539 participants comprised of an intervention cohort of 989 low-income children who participated in the Child Parent Centers (CPC) preschool and kindergarten programs in 1985-1986 and were matched with 550 children who took part in alternative initiatives. In the CLS, children in both groups were matched based on their ability to participate in preschool, race, gender and family socioeconomic status.

The main effects findings indicated CPC participants to have consistently lowered rates of substance misuse since age-16 years and substance misuse with a guilty verdict on drug possession. Of the three outcomes involving smoking, CPC program youth were less likely to be daily smokers. Results from the moderation analysis suggested a broad applicability of the CPC preschool program towards ensuring positive health in the sample. Furthermore, high school completion mediated the effects of preschool at age-24 years accounting for significant decreases in the program's main effects on both measures of substance misuse and daily smoking.
