Gestational diabetes impacts 6-8% of pregnancies worldwide and is associated with both pregnancy complications and lifetime morbidity, as 40-70% of patients progress to diabetes mellitus in their lifetime. Postpartum screening for glucose intolerance is critical to preventing morbidity. Current compliance with the four to twelve-week postpartum screening protocol is low, thereby delaying diagnosis and treatment of glucose intolerance. This quality improvement project aimed to improve postpartum screening compliance at one organization through the implementation of early screening on day two postpartum. Using the Institute for Healthcare Improvement's (IHI) Model for Improvement, and the plan, do, study, act (PDSA) framework, one interprofessional team developed a bundle of interventions including early screening protocols, prenatal and postpartum patient education, and staff and provider education aimed to improve patient counseling. Following the implementation of the early glucose tolerance testing bundle the organization realized a 50.4% improvement in screening compliance from 17.7% in the pre-intervention period to 68.1% in the post-intervention period. Improved screening compliance enhances recognition and treatment of disease, thereby reducing the lifetime risk of developing diabetes mellitus for these at-risk patients.
