Medical errors cost about $29B dollars annually and are the leading cause of death in this country, ahead of breast cancer, AIDS, or motor vehicle accidents. This study examined the relationship between health care provider practice and quality of care within the context of a quality-improvement program. The researcher utilized a qualitative research methodology with content analysis design using NVivo 2.0 to explore historical data in the form of medical record audit documents. Six health care providers participated in a health care quality-improvement program that examined provider health care practice from June 2003 to June 2004. The data from this study identified that provider practice and quality of care can be identified in medical record documentation. The data suggests 5 patterns or themes that emerged as contributing factors in health care provider practice: (a) health care provider participation, (b) positive provider documentation rate, (c) negative provider documentation rate, (d) provider documentation improvement over time, and (e) sustained provider documentation rate.
