Community health organizations (CHOs) in the U.S.A. and China, despite great differences in ideology, economy, and organization patterns in these two countries, demonstrate many similar characteristics which do not fit well into the bureaucratic model. In both countries, CHOs are sponsored by actors whose ultimate concern goes beyond the health of the community population. CHOs are not incorporated into institutions but intensively interact with them. Inside CHOs, hierarchy, role specification, formal qualification and procedures are de-emphasized. Based on the development of CHOs, as perceived from the perspective of CHO organizers, an ideal-type of a new organization model--the "Semi-institutional Arrangement"(SIA) is constructed. A SIA is a mobilization mechanism built to optimize resources in order to meet the needs not fulfilled by regular institutions. Its emergence and development are dependent upon the following conditions: (1) there are functional gaps left behind by regular institutions; (2) there are strong demands to fill these gaps; (3) there is an alleviation of the intervention of the dominant interest groups and aggregates; (4) there is the availability of institutional support; (5) there is the availability of community support; (6) there is the availability of "extra" resources. Eight characteristics of the SIA are identified and explicated: (1) continuous coordination of institutional and de-institutional modes of division of labor; (2) de-emphasis of uni-dimensional hierarchy; (3) integration of formal and informal relationships; (4) emphasis on immediate rather than written communication; (5) emphasis on short-term planning and readiness for spontaneous adjustment; (6) domain legitimization and minimization of inter-organizational conflict; (7) structural de-coupling and restriction of inter-organizational commitment; (8) interpenetration with the community. This semi-institutional mechanism is expected to coordinate the activities of the community and the institutions so that with a limited commitment each side may be able to pursue its own goals. The theoretical significance of the SIA model for organization theory is discussed. Problems and accomplishments of CHOs are assessed. Reasons for and implication of the similarities and divergence of CHO development in the U.S.A. and China are contrasted in the context of the political-economic dynamics in these two countries since the 1940s.
