Preterm delivery is an urgent concern in the United States. Maternal stress has long been postulated as a contributory factor to preterm birth. The measurement of neuroendocrine stress hormones, corticotropin releasing hormone, cortisol and beta endorphins, produced by the placenta and associated with preterm birth, may lead to early identification of women at risk for preterm birth. Stress in pregnancy studies, both with and without neuroendocrine parameters, have not produced consistent results. Eight women who identified themselves as having a violent relationship with an intimate partner were recruited from a rural Midwestern community. They were matched on age, ethnicity, parity, gestational age and personal and family income with non-battered controls. Mid-gestational measures of battering, stress and neuroendocrine parameters did not demonstrate a relationship to preterm birth, however, women in violent relationships did demonstrate neuroendocrine patterns which varied from control women. Women who experience violence in pregnancy represent a sub-group that has not previously been well studied. Stress in pregnancy studies should include screening for violence. Nurses play an important role in improving the status of women and ending violence against women.
