Lack of diabetes self-management skills among patients with Type 2 Diabetes (T2DM)may contribute to uncontrolled blood glucose levels. The project site had a gap in the use of an evidenced-based assessment for diabetes self-care. The purpose of this quantitative, quasi-experimental quality improvement project was to determine if the translation of Schrauben et al's research using the Summary of Diabetes Self-Care Activities Assessment (SDSCA) would impact referral rates to the diabetic educator and reduce fasting blood glucose (FBG) levels when compared to current practice in patients with T2DM. The project was implemented on two inpatient medical-surgical units in an acute care hospital in urban Florida over four weeks. Imogene King's theory of goal attainment was the theoretical underpinning for this project. The total sample size was 29, n=15 in the comparative group and n=14 in the implementation group. Data were extrapolated from the facility's electronic health record. A z-test was run to analyze the referral rates to the diabetic educator demonstrating a statistical and clinically significant improvement in the rates of referrals (z = -4.32, p= .001, 95% CI = [-0.83, -0.31]). A paired t-test was used to analyze FBG levels which were of reduced both clinically and statistically t(14)=2.88, p=.013. Therefore the implementation of the SDSCA may improve referrals to a diabetic educator and reduce FBG levels in this population and setting. Recommendations are to sustain the project and expand it to analyze changes in the hemoglobin A1c levels of the patients.
