This study compared physical, psychosocial, and behavioral outcomes of hospice care and of nonhospice care for terminally-ill patients and their families to determine the efficacy of the hospice-care program objectives. These objectives were, first, to promote physical and psychological comfort to the dying through palliative medical treatment (pain and symptom control), thus, lessening levels of anxiety, depression, and pain; second, to promote quality of life by maintaining the rights of the dying patient, thus ensuring dignity, self-worth, and independence; and third, to promote family adaptation through (a) family crisis intervention and (b) anticipatory guidance related to the physical care of the patient, grief and loss, and community resources, thus, improving the family's preparedness, environment, and coping ability. The methodology consisted of repeated measures of levels of the patient's comfort and quality of life taken on admission and at three weeks and of levels of family adaptation taken at admission, 3-week, and 6-week intervals. Fourteen hospice and 14 nonhospice patients and 9 nonhospice and 8 hospice families were in each group; consequently, the sample size for this study consisted of 28 patients and 17 families. Results indicated that hospice care provided: (1) comfort to the dying patient by reducing pain; anxiety and depression, however, worsened over time. (2) quality of life to the dying patient by maintaining dignity; the patient's perception of independence and self-worth, however, were low. (3) family adaptation through educational preparation and maintenance of the family environment; the family members' strategies for coping and ability to handle conflict, however, lessened over time. There were no significant differences between hospice and nonhospice patients on treatment outcome related to hospice-care objectives. Consequently, hospice care was found to be comparable to nonhospice or traditional medical care. These treatment objectives were evaluated to assess the quality of supportive care in reducing the pain, anxiety, and depression of the patient with cancer. The results suggested that supportive care reduced the patient's pain, but supportive care appeared to escalate the anxiety and depression of the patient. Research methodology appropriate to study the dying patient was established, and implications for the educational psychologist were outlined in the study.
