Introduction: Attention deficit hyperactive disorder (ADHD) and autism spectrum disorder (ASD) are neurobehavioral syndromes that begin during childhood and have long-term clinical and social implications for affected children. Children with ADHD or ASD have complex health care needs that require multiple types of specialized health care services, including motor, behavior, and developmental needs requiring therapy. Despite needing higher rates of health services compared to other children, children with ADHD or ASD commonly experience challenges accessing needed services, therefore impeding their use of the services. Medicaid appears to buffer the health care service costs for children ADHD or ASD, as well as providing comparatively more mental health services than private insurance.Methods: Four waves of the National Survey of Children's Health (NSCH) datasets (2016, 2017, 2018, 2019) was merged for the study. Using the NSCH datasets, multivariate logistic regression analysis was conducted to determine the factors associated with (1) the predictors and prevalence of diagnosed ADHD or ASD among children aged 3-17 years old. Independent Z-test analysis was used to determine differences in the predictors of diagnosed ADHD or ASD by state's Medicaid expansion status. (2) The use of behavioral, pharmacological, or educational treatment services among children with ADHD or ASD aged 3 to 17 years old. (3) school-related behavioral outcomes for children with ADHD or ASD aged 6-17 years old.Result: (1) Predictors of diagnosed ADHD or ASD significantly differed between Medicaid expansion and non-expansion states in three demographic factors: percentage of federal poverty level, primary language spoken at home, and caregivers' highest educational level. (2) Utilization of behavioral health treatment and special education program increased in Medicaid expansion states relative to non-expansion state. Conversely, pharmacological treatment saw a reduction. (3) Medicaid expansion was associated in increased report of missed school days as well as caregivers' perception of conditions' effect on child's ability.Conclusion: This study provided foundational information on the association of Medicaid expansion and health outcomes of children with ADHD or ASD. It yields a further understanding of the influence of Medicaid policies on multiple health outcomes for children with ADHD or ASD.
