Early intervention is known to improve the prognosis for individuals diagnosed with autism spectrum disorder (ASD), leading the American Academy of Pediatrics to publish autism-specific screening guidelines in 2008. These guidelines also referred to as the algorithm, state that all children should be assessed with an ASD-specific screening instrument during their 18 and 24-month well-child visits. However, the rate of ASD screening has historically been low, and few studies have investigated whether physicians' ASD screening practices have changed. Furthermore, there is insufficient evidence on screening disparities at the socio-demographic level, specifically those based on minority and socioeconomic factors. To address this gap, this study had four aims: First, determining AAP screening compliance rates amongst California pediatricians (N = 32); second, comparing pediatrician-reported screening practices with parent-reported access to ASD screening amongst English and Spanish-speaking parents residing in California (N = 176); third, determining whether parents' socio-demographic variables predict access to ASD screening; and fourth, determining whether pediatrician-reported confidence and competence in ASD correlate with their screening practices. The results indicated that while a majority of pediatricians reported routinely screening for ASD, parents largely contradicted this by reporting no access to ASD screening. Access to screening was significantly correlated with mean household income and parent education, while pediatrician confidence and knowledge in ASD also correlated positively with screening practices.
