Food insecurity and limited access to healthy foods are significant contributors to health inequity and are leading social determinants of health. Healthcare screening for unmet social needs, such as food access, is being increasingly implemented in health systems seeking to balance clinical care with social drivers of health. These emerging screening practices are often times a part of intervention programs that address food insecurity by connecting patients with social services and providing medical referrals to community resources and education. This study investigates the prevalence of food insecurity in the patient populations of county-ran health department in North Carolina by measuring screening rates dedicated to identifying this public health issue. Health department clinics report billable services using data extracted from Electronic Health Record (EHR) systems to the North Carolina Department of Human and Health Services (DHHS), Division of Public Health for state-wide aggregation by Medicaid Managed Care region. DHHS data is used in combination with publicly available data describing community access to resources and county-level health indicators. This research highlights factors that contribute to greater susceptibility to food insecurity, suggests process and policy improvement for standardized food insecurity assessment and EHR use for data collection and analysis, and seeks to support state-level healthcare quality assessment.
