Realization that chronic illness was responsible for significant morbidity and nonproductivity in the United States emerged early in the 19th century, shifting research interest toward prevention. Prevention practices are considered important ways to decrease chronic illness and have been performed by public health nurses since the time of immigration in America. Prior to the Patient Protection and Affordable Care Act (ACA) of 2010, preventive care services and health coverage were not equally and widely available to all individuals in the United States, resulting in a rise in chronic illness rates. Key components of the ACA were expansion of health insurance access to millions and provision of affordable preventive care services. Despite this health care advance, a significant number of individuals continue to underuse preventive health services. Health literacy, ethnicity, and poverty are factors that contribute to underutilization of preventive health services. A hermeneutic phenomenology research study was chosen to explore how individuals prioritize decisions regarding preventive care services. Individuals who either have or have declined health insurance will be studied via interviews to explore their true lived experiences with respect to personal health care practices and prioritization. Education regarding health insurance plans, access to care, preventive care services, and how these services impact chronic disease is an important factor necessary to increase community awareness. Understanding the primary reasons underlying the issue can provide an education framework with which nurses, health care providers, and political leaders can initiate positive change.
