The purpose of this study was to identify the needs of spouses of post-9/11 veterans diagnosed with posttraumatic stress disorder (PTSD). There are two general approaches to family interventions: systemic treatments that focus on formal interventions such as marital therapy and support treatments that focus on psychosocial interventions (e.g., skills training). Unfortunately, neither approach has yielded significant insights into the needs of the spouses. The main question for this study asked: How can military spouses who are caring for a PTSD-diagnosed veteran be better served? Three subquestions were included: (a) What services would improve or reduce relationship distress? (b) What services are most helpful to family members in PTSD treatment? and (c) How could prevention efforts be improved to meet the needs of the spouses better? Three data sources used in this study included individual interviews, a focus group, and the researcher field journal. Through qualitative research and purposive sampling, individual and focus group interviews focused on inquiring about services that helped answer the research questions. Seven spouses from a rural veteran population were asked to participate in an action research study. The spouses either participated in an open-ended interview or participated in a focus group. Themes elicited from the individual interviews were posed to the focus group participants and discussed in more detail. The researcher field journal further served to identify themes and was used to triangulate the data. Of the potential participant pool, four participants ultimately participated in the study. All participants were female between the ages of 39-67 years, with three of the four with young children at home. Two participants had spouses from pre-9/11 service, and two had spouses from post-9/11 service. Transcripts were analyzed and coded. All spouses noted a shared experience related to barriers to care and treatment options. They differed on whether informal or formal interventions were most helpful. Ultimately, the spouses agreed that support in either form, formal or informal, was critical to improving marital discord and improving family functioning. This study's conclusion was used to provide veteran spouse groups with ways to reach out to other spouses who care for a PTSD-diagnosed veteran. The action plan promoted education about service intervention and informal support for spouses within the local community. This outreach had implications for social work practice with this population.
