Background. More than half of all shoulder pain complaints have been linked to subacromial impingement syndrome (SIS) and scapular dyskinesia (SD; Tien & Tan, 2014). Literature review. A literature review was performed to evaluate the treatment strategies of shoulder pain due to SIS and SD as well as determine any correlation between shoulder pain and trunk stabilization. Search engines included were PubMed, PEDro, MEDLINE, Google Scholar, and Conchrane Library. A total of 12 articles, including systematic reviews, randomized control trials, and observational studies, were appraised and analyzed. The review of literature revealed limited research on the effects of trunk stabilization as a primary treatment strategy for shoulder pain, SIS, and/or SD. Purpose. The purpose of this study was to evaluate the effects of trunk stabilization as a primary physical therapy intervention for shoulder pain secondary to SIS and SD. Case description. The patient was a 44-year-old male, 10 weeks post-operative right shoulder rotator cuff and type 1 labral repair with subacromial decompression, who complained of intermittent shoulder pain. The patient presented with 3/10 shoulder pain, positive signs of SIS, and SD. Outcomes. Following ten physical therapy treatment sessions, the patient showed improvements in trunk stabilization as well as reduced shoulder pain. Improvements in trunk stabilization were evident by decreased rib flare during shoulder flexion and decreased trunk rotation during stabilization tests. Additionally, scapular and glenohumeral static positioning along with scapular mechanics normalized. Improvements in shoulder and scapular strength were also noted. Discussion. In this case study, there is a positive relationship between trunk stabilization and shoulder pain. As the patient improved trunk stabilization coordination, strength, and endurance, his shoulder pain decreased, and scapular mechanics improved.
