Few studies in the area of empirically supported treatments have examined the relationship between supervision, treatment adherence, and clinical outcome. Functional Family Therapy (FFT) is an evidence-based treatment for at-risk youth and their families. Existing data support the finding that FFT therapists who adhere to the model produce better clinical outcomes. The present study is aimed at learning more about the role of supervision in facilitating therapist model adherence. The goal of FFT supervision is to help therapists know, understand, and apply the treatment model in the way it was intended. Broadly defined for FFT, knowledge focused supervisory interventions facilitate an intellectual and working understanding of the core principles and clinical protocol of the treatment model. Performance based interventions help therapists apply the model to a specific case as they prepare for a family therapy session. The current study was designed to explore the impact of supervisory intervention emphasis (either knowledge or performance) on therapist model adherence to FFT. Specifically, what effect did supervisory intervention emphasis have on the initial status of and change in therapist model adherence over a 4-month study period as newly trained FFT therapists work with family clients? The study used an observational method to rate the content of supervision sessions and the level of therapist treatment model adherence over a 4-month study period. A total of 5 FFT supervisors and 23 therapists comprised the sample for this study. Multi-level modeling was used to analyze the data with a goal of learning more about the relationship between supervisory interventions and therapist model adherence over time. Results indicated that performance focused interventions were negatively associated with the rate of change in treatment model adherence over the study period. This may indicate the importance of fostering a strong conceptual understanding of the model versus telling therapists what to do with a particular family. While the ability to generalize the results of this study to other treatment approaches is limited, it provided valuable information about the role of supervision in facilitating adherence to the FFT model.
