Objective: To evaluate the factor structure, reliability, and validity of the Selective Mutism Sensory Processing Questionnaire (SMSPQ) and document the comorbidity of selective mutism (SM) and sensory processing disorder. Method: A sample consisting of 309 parents of children with SM was recruited via various social media pages and websites related to SM. The children were between the ages of 3 and 14 inclusive; 70% were girls and 30% were boys. SM diagnoses were verified by parental acknowledgment that the child had been diagnosed by a physician, speech-language pathologist, or mental health treating professional. Participants completed four questionnaires: Child Behavior Checklist, Selective Mutism Questionnaire, Sensory Profile 2, and the SMSPQ. An exploratory factor analysis was conducted to investigate the dimensionality of the SMSPQ. Internal consistency, construct validity, and reliability were also examined. Pearson product-moment correlations were conducted to determine relationships based on setting, SM severity, sensory processing, and additional comorbid disorders. Results: Results provided clear support for the psychometric properties of the SMSPQ. The examination of factor structure revealed an eight-factor solution that had excellent internal reliability and consistency in the domains of Food, Change/Transitions, Vestibular, Physical Risks, Termination of Activities, Proprioception, Auditory, and Fears/Worries/Anxiety, as well as in three settings (school, home, public), that reflected specific SM characteristics. Significant positive correlations between SM and sensory processing challenges, specifically in the areas of oral sensitivities, auditory processing, emotional regulation, and anxiety, were revealed. Conclusions: Sensory processing disorder is a documented comorbidity of SM. The SMSPQ will provide clinicians from various professions a method for screening sensory processing challenges without the necessity for an occupational therapy evaluation. The SMSPQ should prove useful in initial evaluations, treatment planning, and monitoring progress. With this information, clinicians should consider an intervention focus that is holistic and based on more than the outward symptom of not speaking. A multidisciplinary team, that includes a mental health treating practitioner, a speech-language pathologist, and an occupational therapist would best meet the needs of most children with SM.
