Purpose: The purpose of this case study was to determine if average estimates of sound pressure level (SPL), fundamental frequency (f0), cepstral peak prominence (CPP), and H1-H2 would vary systematically in a patient with bilateral vocal fold nodules who experienced [1] worsening of her lesions, [2] surgical removal of her lesions, and [3] successful vocal rehabilitation post-voice therapy. Methods: The patient's voice was monitored for 4 weeks. Week 1 occurred before any intervention, week 2 occurred after her lesions worsened, week 3 occurred post-surgery, and week 4 occurred after voice therapy. Change in average SPL, f0, CPP, and H1-H2 were evaluated using Cohen's d, where any d ≥ 0.40 was considered clinically relevant. Results: The patient's average vocal behavior showed significantly increased SPL during week 3 and week 4 compared to weeks 1 and 2, decreased f0 uring week 2 compared to all other weeks, and decreased H1-H2 during week 4 compared to week 1. There were no statistically significant differences in average CPP between any two monitored weeks. Conclusions: Whereas surgery and voice therapy appeared to be associated with an increase in average SPL and decrease in average H1-H2, worsening of the vocal fold lesions was only associated with decreased average f0.
