Time to task failure (TTF) on the Sorensen test predicts low back pain (LBP) and is a strong discriminator of those with and without LBP; however, the mechanisms driving TTF may vary in those with and without a history of recurrent LBP. Performance likely depends upon a complex interaction of physiological factors, such as anthropometrics, strength, and the median power frequency slopes of the trunk and hip extensors, and psychological factors, such as pain-related fear, self-efficacy, and motivation. Virtual reality video games have been used to improve tolerance of burn debridement and chemotherapy treatments and has been shown to encourage physical activity in kinesiophobic LBP populations. To determine the physiological and psychological predictors of TTF on the Sorensen test and a variation of the Sorensen test using a virtual reality video game, 48 sex, age, and BMI matched participants (24 Healthy, 24 LBP) completed psychological surveys, maximal strength assessments, the classic Sorensen test, and the virtual reality Sorensen test. Self-efficacy and trunk mass appear to be consistent predictors of performance, while the median power frequency slopes of the trunk and hip extensors appear to be predictive of performance only in individuals without a history of recurrent LBP.
