Quality of life changes for cancer patients were measured in a fully randomized, controlled study where differential treatment effects were compared for three group psychotherapeutic interventions: Mental Imagery, Group Therapy and Laughter/Humor.

Participants with an active diagnosis of cancer of any stage and type were randomly assigned to one of the four groups (n = 43), the sample comprising 31 females and 12 males, with a median age of 50.6 years. Treatment meetings involved 2-hour, biweekly sessions for a period of one month. The research design used was a 4 x 2 analysis of variance (ANOVA) mixed, between and within fully randomized factoral design. Factor one was treatment, factor two the pre- and post-intervention assessments. A minimum of n = 10 were assigned to each group.

Psychometric instruments used were the Functional Living Index-Cancer (FLI-C), the Millon Behavioral Health Inventory (MBHI), the Profile of Mood States (POMS) and the Family Environmental Scale (FES).

It was hypothesized that all treatments (excluding control) would produce statistically significant decreases in total mood disturbance (TMD), depression, anxiety and subjective pain levels, with the more superior improvements on all dependent measures predicted for the Laughter/Humor group.

Significant decreases in interpersonal disruption was also expected for the group therapy treatment as well as the greatest reductions in chemotherapy-associated nausea for the Imagery group.

Results partially confirmed these forecasts, with statistically significant decreases in anxiety (p =.05) for all groups and a trend toward a significant main effect for depression (p =.071). The Laughter/Humor group did not show superior improvements on all dependent measures when compared with all other group treatments, as hypothesized.

Multiple comparisons using Fisher's Protected Least Significant Difference test (PLSD) revealed that the Laughter/Humor group had significantly greater decreases in Pain Treatment Responsivity at post-intervention than the control group. Interpersonal conflict scores were elevated to a highly significant degree on post-test measures for all treatments (p =.005). This is explained in terms of psychodynamic/psychoanalytic theory and offers evidence that brief, short-term therapy treatments of this type may be contraindicated for some medical populations.
