This research examined an alternative intervention for transformation of depressed mood in pregnant women. Prenatal depression is a mood disorder that can have long-range effects on the fetus and is detrimental to the mother. It is a significant problem that affects women from a variety of socioeconomic and educational backgrounds. The purpose of this concurrent mixed method, exploratory multiple case study, was twofold: (a) to explore in depth, the lived experience of pregnant women who have depression; and (b) to examine the efficacy of using the HeartMath Personal Stress Reliever (PSR) and Quick Coherence breathing method, providing heart rate variability biofeedback (HRVB) to transform their experience. Forty-one women, ages 18 to 40 years, were recruited locally and online from Facebook and craiglist.org. Participants were not on prescription medication or receiving therapy. Participants self-selected the experimental group (Group 1) or the control group (Group 2), each containing a minimum of 20 participants. Group 1 used the PSR and Quick Coherence technique daily over 4 weeks (an instructional video was posted on a designated website). Group 2 experienced their pregnancy as usual during the 4-week period. Quantitative data were collected from 2 online surveys: the Edinburgh Postpartum Depression Screening Scale (EPDS) and the Postpartum Depression Screening Scale (PDSS) taken as pretests and after 4 weeks, posttests. The qualitative portion of the study included 4 case studies using semi-structured interviews and written answers to 7 open-ended questions. Quantitative data were statistically analyzed using SPSS statistical software. Qualitative data were analyzed using thematic content analysis revealing 8 themes: Physical, Social, Coping, Relationships, Negative Emotions, Positive Emotions, Life Situations, and Self-Awareness. Quantitative findings showed that Group 1 experienced a significant drop in depression as evidenced by EPDS scores, with no significant change in Group 2. HRVB using the PSR device was shown to be an active intervention that introduced feelings of empowerment and self-control. HRVB showed great potential as an alternative treatment for women with prenatal depression. Any intervention that improves the health and well-being of the pregnant woman may also enhance the health and well-being of her unborn child.
