This dissertation examines the relationship between metaphor and ideology in the public rhetoric of leading Democrats and Republicans in the U.S. Congress, who engaged in debate on the policy issue of universal health coverage during the early years of the Clinton administration (1993-1994). When Clinton first outlined the policy issues in September 1993, the Republicans, led by Senator Robert Dole, appeared reconciled to the passage of this public policy. Later, the Republican leadership decided to oppose the bill and a contentious debate ensued.

The study focuses on metaphor in the political communication that took place in the problem-setting and policy formulation stages of legislation. Metaphor was selected as the focus of the dissertation because of the insights metaphorical images can offer into the ideology of the rhetors and their implied auditors. In the debate, both major factions used journey, disease, machine, narrative, balance and force metaphors. Although they used identical metaphorical bases, their images were oppositional. The study introduces a new theoretical concept, oppositional correlatives, as a contribution to rhetorical criticism. It gives the critic a useful term in examining metaphors that issue from the same conceptual base, but have contradictory meanings. The study suggests that the effectiveness of a metaphor over time is partially dependent on its ability to protect itself against opposing metaphors. This extends rhetorical theory concerning the way metaphors operate. The study also demonstrates how a series of metaphors can construct an argument. The metaphors in the debate provide insight into oppositional worldviews concerning the issues of the relationship of the U.S. government, its citizens and the limits of government power. The history of political communication around national health insurance shows that this has been the issue concerning health care reform from the beginning of this century. The metaphors examined in this study of public argument encapsulate questions about the priority of individual/community, voluntary/obligatory, rights/responsibilities and power/limitations. The study reveals opposing tensions about the American way of providing health care.
