The care of persons with cognitive deficits is specialized, but little is known about the efficacy of dementia care programs in nursing homes. This study used a retrospective longitudinal quantitative approach to examine the effect of an Allen cognitive approach to nursing home care. Grounded in the cognitive disabilities model, and using the Allen cognitive levels, the Integrated Cognitive Program is an occupational therapy driven, interdisciplinary approach to caring for people with cognitive deficits. The Integrated Cognitive Program enables nursing home staff to identify the abilities of residents and provide a more holistic approach to care. This study compared baseline de-identified data from resident-specific Minimum Data Sets (MDS) and Quality Indicator Reports for the year prior to implementation of the Integrated Cognitive Program to data 5-months, 8-months, and 11-months post implementation of the program. The study cohort consisted of the 31 residents who had resided in the study nursing home for the 23-months of the study. All subjects were Caucasian. The results of this study indicate that the Integrated Cognitive Program significantly reduced the number of quality indicators triggered for behavioral symptoms affecting others, and significantly reduced the total number of quality indicators for residents who had an average of 2.67 or more quality indicators prior to the implementation of the Integrated Cognitive Program. Although the sample size and the homogeneity of ethnicity decrease the ability to generalize, these results contribute to closing the gap in the literature on the effectiveness of programs for nursing home residents with cognitive deficits. These results support the implementation of Allen cognitive based programs to improve the care and quality of life of nursing home residents.
