The effectiveness of an occupational therapy intervention in facilitating intrinsic motivation among individuals with psychiatric illness was investigated in the study. The occupational therapy intervention was implemented according to a set of practice guidelines that the researcher developed to resolve motivational deficits, which were the passive or coercive behaviors resulting from cumulative frustration experiences in the past. The intervention had a theoretical base derived from the self-determination theory and the learned helplessness theory and contained strategies of autonomy support and moderate structure. The study was carried out in six psychiatric hospitals in Taiwan. The research design was a mixed-effects nested design, involving one experimental group and one control group, four types of motivational status, and multiple therapists. The dependent variable was the amount of change in intrinsic motivation before and after the treatment. Ninety-nine clients from day treatment or rehabilitation units of the participating hospitals participated in the study. The experimental group received the proposed intervention for 12 weeks, with one-hour individual treatment sessions twice a week, while the control group received the standard occupational therapy services. Results of the study showed that the treatment effects varied among types of motivation. The proposed occupational therapy intervention intrinsic motivation among clients with motivational deficits, including the control-oriented and the impersonal-oriented, as well as the autonomy-oriented. But the intervention was not as helpful for clients with mixed type of motivation. The improvement in intrinsic motivation yielded a corresponding change observed in social behavior. The relationships between improvement in intrinsic motivation and usage of intervention strategies for each motivation type were unable to be determined due to insufficient sample sizes. Preliminarily, the categorization of intervention strategies was adequate but some refinement is needed. It is concluded that the proposed occupational therapy intervention has a potential to be used in clinical practice. Further research is needed to investigate other aspects of adequacy of this intervention.
