The debate surrounding the categorical versus dimensional classification of mental disorders, particularly regarding personality disorders, is longstanding and well-documented. To develop a valid diagnostic system and effective treatments, it is important that we understand the fundamental structure of personality pathology, namely, whether personality disorder is categorical or dimensional in nature. New statistical techniques allow for direct empirical comparison between dimensional and categorical models. In the present study, we compared the relative fit of latent class (categorical), latent trait (dimensional), and factor mixture (hybrid) models of the ten DSM personality disorders in a nationally representative sample of approximately 43,000 participants. Multidimensional latent trait models provided the best fit to the data in each case, indicating that personality disorders are not qualitatively distinct disorders but rather represent extreme variants of normal personality traits.
